These Terms and Conditions (“Terms”) govern your access to and use of services provided by Practice with Purpose, LLC, a Texas‑based, Nurse Practitioner‑owned integrative health and medical weight management practice (“Practice,” “we,” “us,” or “our”). By accessing our website, scheduling services, signing intake documents, or participating in care, you agree to these Terms.


1. Scope of Services

Practice with Purpose, LLC, provides integrative, functional, preventive, and primary care‑based services that may include medical weight management, metabolic health optimization, lifestyle and nutritional counseling, functional wellness testing, prescription management, preventive care, and health education.

All medical services are provided by a licensed Nurse Practitioner acting within the scope of practice defined by Texas law and applicable regulations. Services may be offered in‑person, via telehealth, or through hybrid models based on clinical appropriateness and licensure requirements.


2. Provider‑Patient Relationship

A provider‑patient relationship is established only after:

  • Completion of required intake forms and consent documents
  • Review of medical history and health information
  • Clinical acceptance into care
  • Completion of an initial clinical visit with the Nurse Practitioner

Educational content on our website, social media, emails, or marketing materials does not constitute medical advice and does not establish a provider‑patient relationship.


3. Clinical Judgment and Individualized Care

All care is individualized and based on clinical assessment, patient‑reported history, diagnostic data, and evidence‑informed integrative and conventional approaches. Treatment plans may include lifestyle modification, nutritional strategies, medications, supplements, and referrals as clinically appropriate.

Treatment plans may be modified, paused, or discontinued at any time for safety, lack of benefit, regulatory requirements, or patient preference. No specific outcomes are guaranteed.


4. Functional and Integrative Wellness Testing

Functional wellness testing may include specialty laboratory assessments such as metabolic, hormonal, micronutrient, gastrointestinal, inflammatory, genetic, and food sensitivity testing.

You understand and agree that:

  • Functional wellness tests may not be considered standard diagnostic tests in conventional medicine
  • Testing is often self‑pay and typically not covered by insurance
  • Results are used to support clinical insight, wellness planning, and risk assessment rather than standalone diagnosis
  • Results must be interpreted within the broader clinical context
  • Abnormal findings may require confirmatory testing, referral, or conventional medical evaluation

Laboratory testing is performed by independent third‑party laboratories. The Practice is not responsible for laboratory errors, specimen handling issues, processing delays, or lab‑specific policies.


5. Medical Weight Management and Metabolic Care

Weight management services may include nutritional counseling, behavioral strategies, physical activity guidance, metabolic assessment, and pharmacologic therapy when clinically appropriate.

You acknowledge that:

  • Weight loss results vary between individuals
  • Weight regain is possible despite adherence
  • Lifestyle modification remains a core component of treatment
  • Treatment may be discontinued if risks outweigh benefits

6. Anti‑Obesity and GLP‑1/GIP Medication Disclosure

Anti‑obesity medications, including GLP‑1 and GLP‑1/GIP receptor agonists, may be prescribed based on clinical criteria and individual risk assessment.

You acknowledge:

  • These medications may cause side effects including, but not limited to, nausea, vomiting, diarrhea, constipation, abdominal discomfort, and appetite changes
  • Serious adverse events, though uncommon, may occur
  • Some prescriptions may be off‑label
  • Medications may be discontinued due to side effects, lack of efficacy, or safety concerns
  • Medication shortages, insurance coverage changes, or pharmacy availability may impact access


Compounded medications, when used, are prepared by licensed third‑party compounding pharmacies. The Practice does not manufacture medications and is not responsible for pharmacy‑related issues.


7. Supplements and Integrative Therapies

Dietary supplements, nutraceuticals, and integrative therapies may be recommended to support wellness goals. You acknowledge:

  • Supplements are not FDA‑approved to diagnose, treat, cure, or prevent disease
  • Quality and sourcing may vary between manufacturers
  • Potential interactions with medications are possible
  • You are responsible for disclosing all supplements you take


8. Telehealth Services

When telehealth services are utilized:

  • You must be physically located in a state where the Nurse Practitioner is licensed at the time of the visit
  • Telehealth has inherent limitations compared to in‑person care
  • Technical failures may impact service delivery
  • You are responsible for ensuring privacy on your end


9. Cash‑Pay Practice and Fees

[Practice Name] operates as a cash‑pay practice. We do not bill insurance for services unless explicitly stated.

  • Payment is due at the time services are rendered
  • Fees for visits, programs, testing, medications, and supplements are disclosed in advance whenever possible
  • Insurance reimbursement is the patient’s responsibility if applicable
  • No refunds are provided for completed services, ordered tests, dispensed medications, or missed appointments


10. Appointments, Cancellations, and No‑Shows

  • Appointment policies are provided at scheduling
  • Late cancellations and no‑shows may result in fees
  • Repeated missed appointments may result in termination of care


11. Patient Responsibilities

You agree to:

  • Provide accurate, complete, and updated health information
  • Disclose medications, supplements, allergies, and diagnoses
  • Follow agreed‑upon treatment plans or communicate concerns
  • Report side effects, changes in condition, or adverse events promptly

Failure to meet these responsibilities may compromise care and safety.


12. Not Emergency or Acute Care

Our services are not designed for emergency or urgent medical needs. In the event of a medical emergency, call 911 or seek immediate emergency care.


13. Privacy and Confidentiality

Protected health information is maintained in accordance with HIPAA and applicable state and federal privacy laws. Third‑party vendors and laboratories maintain their own privacy and data handling policies.


14. Termination of Services

The Practice reserves the right to terminate the provider‑patient relationship for reasons including non‑payment, non‑adherence, inappropriate behavior, misuse of medications, or clinical determination that care is no longer appropriate. When feasible, reasonable notice and transition guidance will be provided.


15. Limitation of Liability

To the fullest extent permitted by law, [Practice Name] shall not be liable for indirect, incidental, or consequential damages arising from participation in services, testing, medication use, or website access.


16. Governing Law

These Terms are governed by the laws of the State of Texas, without regard to conflict‑of‑law principles.


17. Updates to Terms

We may revise these Terms at any time. Continued use of services constitutes acceptance of updated Terms.
